dc.description.abstract ABSTRACT In the K3, there are methods to identify, analyze, and evaluate risk factors in related projects. The analysis used to systematically determine the causes of workplace accidents is the Fault Tree Analysis (FTA) method. The FTA method is a tool used to identify risks that contribute to the occurrence of failures. It is expected that by analyzing the risk of workplace accidents in this project and addressing them, the results will be very useful for the implementation of similar projects in the future. Considering the vulnerability of occupational accident and health risks in a project, it is therefore necessary to conduct an analysis to identify workplace accident risks as well as the responses to those risks that occur in the Bhayangkara Sari Asih Hospital Project by using the Fault Tree Analysis (FTA) method. The research method used in this study is descriptive research. Another definition of descriptive research is a form of research aimed at describing existing phenomena, whether natural phenomena or human-made phenomena. The research approach used in this study is the Mixed Method, or combined research. Mixed method research is a type of research that integrates two research methods, namely quantitative and qualitative, which are combined and integrated to form new findings and draw conclusions. Based on the results of the analysis using the probability and impact matrix, there are 54 identified accident risks consisting of 28 accident risks categorized as low, 17 accident risks categorized as medium, and 3 accident risks categorized as high. The three accident risks categorized as high are: workers falling from heights, falling materials from heights during reinforcement work, and falling materials from heights during Pier Head work at the Bhayangkara Sartika Asih Hospital project in Bandung. The causes of these accident risks are divided into four factors, namely human factors, work method factors, equipment and material factors, and environmental factors. The risk responses based on the results of the interviews include handling and preventive actions that can be carried out through human factors (Man), method factors (Method), equipment and material factors (Machine), and environmental factors (Mother of Nature).
